New Patients
All new patients must have current X-rays or MRI, taken within 6 months of your visit. If you do not have either, please come to your visit 30-45 minutes before your scheduled appointment so we can send you for imaging. All new patients should plan on coming 15 minutes early to provide our office with information. To save time on the day of your visit you may print out the forms bellow, fill them in, and bring them with you along with a list of current medications and your insurance card. If you have an HMO, we require a referral from your primary care physician before being seen in the office.

Current Patients
Patients needing refills of any medication, please plan ahead. The office policy on prescription refills is during business hours only, 8am-5pm weekdays. Most of the prescriptions Dr. Prpa writes need a written prescription each time they are filled making it difficult to accommodate after hours. When calling during business hours for refills please have the prescription name and pharmacy phone number available. In the case of an urgent matter, our answering service is able to reach us 24 hours a day.
